Question:

A 46 year-old white man presents with a 3-week history of what starts as a freezing feeling in his sinuses which went to his chest within 5 seconds and lasts 5-10 minutes, usually exertional. When these episodes continued he came to the ED.

Unknown risk factors and no other known comorbidities.

Answer:

Story good and no other reason for an increased cTn.  Could be a “late presenter” on the downslope of time-concentration curve (see below) which is much slower than the upslope – suspicious of late AMI.
